1. Respect & Sportsmanship

• Treat coaches, teammates, opponents, officials, and facility staff with respect at all times.
• No bullying, taunting, name-calling, or harassment of any kind will be tolerated.
• Demonstrate good sportsmanship whether winning or losing.

2. Attendance & Punctuality

• Arrive at least 10 minutes before your scheduled session.
• Notify your coach in advance if you will be absent or late.
• Excessive unexcused absences may affect program standing or eligibility.

3. Effort & Attitude

• Give full effort during all sessions. Your development depends on your commitment.
• Accept coaching instruction positively. Disagreements should be addressed calmly and privately.
• No foul language, disruptive behavior, or distracting conduct during practice.

4. Facility & Equipment Use

• Athletes are responsible for the equipment issued to them. Lost or damaged equipment may require replacement at the family's expense.
• No food or drink (other than water) on courts, fields, or gym floor without permission.
• Clean up personal belongings before leaving. VSC is not responsible for lost or stolen items.
• Use of cell phones is prohibited during active practice sessions.

5. Safety

• Report any injury, illness, or unsafe condition to a coach or staff member immediately.
• Athletes must not participate if injured or ill without medical clearance.
• No athlete may remain in the facility unsupervised.

6. Social Media & Online Conduct

• Athletes represent Vitalis Sports Center online. Negative posts about teammates, coaches, officials, or rival teams are prohibited.
• Online harassment may result in suspension or removal from programs.

• Speak respectfully to all coaches, staff, officials, and other families at all times.
• Refrain from coaching your child from the sidelines during practices or sessions.
• Communicate concerns directly and privately with the coach — not during or immediately after a session.
• Support your child's effort and attitude regardless of performance outcomes.
• Ensure prompt drop-off and pickup. Late pickup fees may apply.
• Inform Vitalis Sports Center immediately of any changes to emergency contacts, medical information, or custody arrangements.
• Unpaid tuition or fees may result in suspension from participation until resolved.

Vitalis Sports Center reserves the right to issue disciplinary action for any violation of these rules, including but not limited to:

• Verbal warning from coach or staff.
• Written notice to parent/guardian.
• Temporary suspension from participation.
• Permanent dismissal from programs without refund.

Serious incidents involving violence, threats, substance use, or illegal activity will result in immediate removal and may be referred to appropriate authorities.

1. Nature of Activity & Inherent Risks

Participation in sports training, conditioning, gymnastics, basketball, and all other physical activities offered at Vitalis Sports Center involves inherent risks that cannot be eliminated regardless of the care taken. These risks include, but are not limited to: physical injury (sprains, fractures, dislocations, concussions); cardiovascular events, heat illness, dehydration; injury caused by contact with other athletes, equipment, or facility surfaces; psychological stress associated with competitive sport environments; and illness, including communicable diseases.

2. Assumption of Risk

I, as Parent/Guardian, voluntarily and knowingly assume all risks associated with my child's participation in any activity, program, event, or session offered by or affiliated with Vitalis Sports Center, both on and off VSC premises.

3. Release & Waiver of Liability

In consideration of my child's participation, I hereby release, waive, discharge, and covenant not to sue Vitalis Sports Center, its owners, directors, coaches, employees, volunteers, and agents from any and all liability, claims, demands, actions, and causes of action arising out of or relating to any loss, damage, or injury (including death) that may be sustained by my child or by any property belonging to my child.

4. Indemnification

I agree to indemnify, defend, and hold harmless Vitalis Sports Center and all affiliated parties from any and all claims, actions, damages, liability, and expenses (including legal fees) arising from or in connection with my child's participation in VSC activities.

5. Medical Authorization

In the event my child requires emergency medical attention and I cannot be reached, I hereby authorize VSC staff to call 911, consent to emergency medical treatment by licensed medical personnel, and transport my child to the nearest appropriate medical facility. I agree to assume all costs associated with such treatment.

6. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania.
